The following is a list of materials that you need to take with you:

Your Certified Birth Record
2 Passport Photos
Photocopy of your Picture Drivers License (Front & Back)
Proof of Name Change (if any)
Proof of Departure (airlines ticket or confirmed/paid itinerary)
(2 copies are needed - 1 inside of the package and 1 outside of the package)
Driving Across the Border Letter: If you are driving you will need to write a letter giving details that you plan to drive across the United State border between Mexico and Canada. Use this example in order to compose this letter.
Copy of Hotel Reservation in Canada or Mexico
ACE Passport Services Letter of Authorization
(2 copies are needed - 1 inside of the package and 1 outside of the package)

$115.00 persons age 16 and over
(2 checks: $90.00 Dept. of State $25.00 to Passport Agency)
$100.00 persons age 15 and under
(2 checks: $75.00 Dept. of State , $25.00 to Passport Agency)
Cash, Credit, Check or Money Order are accepted.
