Effective 09-26-05 the Department of State has made it mandatory that all passport applications first be completed online using the 2D bar code application which is available from the Department of State's website.

2. As a Rush Passport Applicant...
After completing the form online, you must appear at the post office or government office which is a passport acceptance office (post office which accepts passport applications or county clerk's office) to have your information verified and your signature witnessed on the passport application.

3. The following is a list of the information that you will need to send to
ACE Passport Services:

A.Your Certified Birth Record.
B. 2 Passport Photos.
C. Photocopy of your Picture Drivers License (Front & Back).
D. Proof of Name Change.
E. Proof of Departure (airlines ticket or confirmed/paid itinerary).
(2 copies are needed - 1 inside of the package and 1 outside of the package).
F. ACE Passport Services Letter of Authorization (PDF Format) to Transport Documents (A.K.A. Courier Authorization Letter) (2 copies are needed - 1 inside of the package and 1 outside of the package).
 G. Correct Application Fee: (Effective 07-13-10).
$195.00 persons age 16 and over.
(2 checks:$170.00 Dept. of State $25.00 to Passport Agency).
$165.00 persons age 15 and under.
(2 checks: $140.00 Dept. of State , $25.00 to Passport Agency).
G. Cash, Credit, Check or Money Order are accepted.

4. The information will be sealed in a 8 x11 postal envelope, take the sealed envelope and put it inside a FedEx envelope, fill out the airbill according to the sample shown.

5. Ship it directly to ACE Passport Services. (call Fedex at 1-800-463-3339 for detail instructions)
