Click here to go to the Department of State Passport Wizard Effective 09-26-05 the Department of State has made it mandatory that all passport applications first be completed online using the 2D bar code application which is available from the Department of State’s website.
After completing the form online, you must appear at the post office or government office which is a passport acceptance office (post office which accepts passport applications or county clerk’s office) to have your information verified and your signature witnessed on the passport application.

The following is a list of materials that you need to take with you:
⬧Complete the application form DS-11 from U.S. Department of State Passport Wizard.
You must make 2 copies of the DS -11 application form. 1 copy inside the sealed package and 1 copy attached to the sealed package.
(The Philadelphia Passport Agency requires all new expedited passports that are sealed by a passport acceptance facility to be accompanied with a second copy of the application to prove that the 2D bar code was placed on the passport application)
- Your Certified Birth Record. (Texas California, Arizona New Mexico residents must have the Long form birth certificate. This form includes the parent’s information and the hospital where the birth occurred.)
- 2 Passport Photos
- Photocopy of your Picture Drivers License (Front & Back)
- Proof of Name Change (if any)
- Proof of Departure (airlines ticket or confirmed/paid itinerary)
(2 copies are needed – 1 inside of the package and 1 outside of the package) - ACE Passport Services Letter of Authorization
(2 copies are needed – 1 inside of the package and 1 outside of the package) - Correct Application Fee: (Effective 01/26/2024)
$244.53 for persons age 16 and over –Check or money order are accepted. Note: After the information is sealed in an 8 x11 postal envelope by the passport acceptance clerk, take the sealed envelope and put it inside a FedEx envelope, fill out the air bill and ship it directly to Ace Passport Services.
