“List of Identities”
If you are applying for a new 1st time passport you need to show proof of identification. A valid drivers license or state issued photo ID from any state is required. If you do not have a valid drivers license or state issued ID you may submit (3) of the following forms of identification or public records which are older than 5 years.
You may send copies of any personal documents that you have which do not appear on this list. Any type of document which is expired such as an identification card or license will also be considered. You should also complete the Supplemental Information Questionnaire (pdf form).
Please send as many identification items as possible.
1) Work Identification Card
2) Government Identification Card (federal, state, municipal)
3) Student Identification Card (school or college)
4) Drivers License (expired or valid)
5) Military Identification Card
6) Military Discharge (DD-214)
7) Selective Service Registration card and/or classification
8) Union Membership Card
9) Professional License (pilot, nurse, beautician, x-ray technician)
10) Medicare card
11) Welfare card
12) Marriage Certificate
13) Divorce Decree
14) Grammar School Diploma and/or Transcript
15) High School Diploma and/ or Transcript
16) College Diploma and/or Transcript
17) Trade or Technical School Diploma and/or Transcript
18) Credit Cards with photo on card
19) Insurance papers
20) Expired Passport(s)
21) Baptismal Certificate
22) Voter Registration Card
23) Vehicle Registration and/or Title
24) Income Tax records (at least 5 years old)
25) W-2 forms (statement of earnings)
26) Hunting / Fishing License
27) Mortgage/Loan Contracts
28) Library Cards
29) Medical/Hospital records
30) Census Report
NOTE: Affidavit of Identifying Witness (Form DS-71 ) the witness must be citizen or permanent resident of the U.S. and must be able to present acceptable identification as well as certify that he/ she has known the applicant for at least 2 years. The witness must be present with the applicant to complete and execute an affidavit before the same agent who adjudicates the application.
